Hospital to Home Preparation for Seniors- Eldercare professionals know better than anyone that older adults who are hospitalized sometimes return home in a significantly altered state. Alternately, they may not return home at all. Based on compelling new research, in this web seminar presenter Molly Carpenter discusses the risk of hospitalization for older adults.
